Service notes for individual #1 were not maintained for 2/28/20.The chief executive officer shall be responsible for the administration and general management of the facility, including the following: Implementation of policies and procedures.Why is the Regulation Important: The chief executive officer shall be responsible for the administration and general management of the facility, including the following: Implementation of policies and procedures. *This regulation is important because it shows that the leaders of this organization want the Supervisors/Program Specialists to be aware of policy and how it pertains to the documentation of services provided to our participants. What Happened: Service notes for individual #1 were not maintained for 2/28/20. Why did it Happen: CPS newly hired Coordinator/Program Specialist was still going through the mentoring process and was not responsible yet to review daily notes. The daily review of progress notes was missed due to an oversight on the part of the CPS Supervisor on not completing this daily task. The individual left early on this date due to not feeling well. Staff assigned to this individual also left early but the staff did not follow the end of the day task routines including daily progress note completion. What do we do right now? CPS Program Supervisor has been retrained in 55 PA Code Chapter 2380. (Attachment #1) How do we prevent it from happening in future: As a preventative measure, CPS Coordinator/Program Specialist will review notes daily. (Attachment #2 and #3). CPS Supervisor will review all notes on a weekly basis. The DSP assigned to this individual and responsible for completing the note will be counseled on ensuring all documentation duties are completed prior to her leaving the shift. In addition all staff will be retrained on the importance of completing documentation. (Attachment #4). Moving forward, our process has always been and will continue to be that the Coordinator will review individual Service notes daily to ensure they have been completed for each individual. The Program Supervisor/Specialist will continue to review notes weekly to ensure that they have been completed and checked. Both the Coordinator and the Program Supervisor/Specialist will initial and date each note to confirm these steps were followed. CPS is temporarily closed due to COVID-19.
