6400.66On 3/23/22 at 11:25 am, the light at the doorway leading to the outside of the home from game room was found inoperable.Rooms, hallways, interior stairways, outside steps, outside doorways, porches, ramps and fire escapes shall be lighted to assure safety and to avoid accidents. A local contractor evaluated and fixed the back flood-flood lights on 3-30-22. One bulb was replaced on the single light and one bulb was replaced on the double light. The contractor believed the power surge on the morning of 3-23-22 may have caused the bulbs to blow. Both lights are now operable and there is no electrical damage. 03/23/2022 Implemented

A training was held on 1-17-2017, including the Program Specialists, Program Assistant and the Program Director, to review the physical form and the requirements for completing all sections including the health maintenance section. When an Individual is taken for a physical, the physician will be instructed to complete all sections of the physical form. If there are no recommendations to be made for a section it cannot be skipped over. Any section that does not require attention at the time of the visit must be marked with an NA. The Program Assistant will be responsible for a second check of all physicals to ensure all sections are completed. No physical form will be excepted unless all sections are filled out. Immediately, all staff persons responsible for assisting individual in physical examinations shall be trained as to what is included in physical examinations as per 6400.141(c)(1)-(15) and the that not any required areas shall be left blank. Documentation of training shall be kept. Within 1 week of competition, the program assistant shall review all individuals' physical examination and immediately obtain missing information from the completing physician. At least quarterly for 1 year, the program director shall review a 25% sample of individuals' physical examinations to ensure all required area of physical examinations are addressed.
