On March 15, 2023, the physical inspection took place that recognized the water temperature being 122.5 degrees. Maintenance was contacted and adjusted the water temperature to ensure that it did not exceed 120 degrees. For the remainder of the month, the program staff are checking the water temperature daily at all accessible sources to ensure it does not exceed 120 degrees. See Attachment #11 for proof of the water being adjusted and tracking of ongoing water temperatures for the month. |

This occurred because the Program Specialist and Assistant Director failed to adhere to this regulation.

At the time of the inspection, there was snack food locked and not accessible to the individuals.
This occurred because the Program Supervisor and Program Specialist failed to ensure that all food was accessible to the individuals.
Immediately following the recognition of snacks being locked, the Program Supervisor moved all the snacks to an unlocked area, ensuring they were accessible to the individuals. See attachment #10.
On 3/20/2023, Program Specialists were retrained on the individual's rights to have access to food. |
