The regulation is important because it provides standard policies and routine practices to minimize the risk of illness, infection, or injury and provide for a dignified living environment. At the time of the inspection on 1/31/24, the exhaust fan in the bathroom connected to the staff office was covered in a thick layer of dust. The violation for the thick layer of dust in the bathroom is due to staff not routinely cleaning the bathroom. The Supervisor of the program as well as the Manager failed to ensure that staff are regularly cleaning the whole bathroom. The dust was cleaned and removed from the exhaust fan on 1/31/2024 following the inspection. See Attachment #2 for proof. Following the inspection, the Program Specialists did a walkthrough of each program to ensure that regulation 6400.64(a) was met. At the time of the walkthrough, no other programs were found to have thick layer of dust found on their exhaust fans. All programs were clean and sanitary during the walkthrough, except for urine smell in one of the programs, which is addressed in a different plan of correction. 03/04/2024 Implemented

6400.112(e)An asleep drill was last held on 1/16/16.A fire drill shall be held during sleeping hours at least every 6 months. A sleep drill occurred on Oct 4, 2016. The next sleep drill is scheduled to occur in April. This is on the Program Specialist calendar to ensure that staff complete it. See attachment H In additional, a monthly paperwork checklist was created that documents when fire drills are turned in and reviewed. Program Managers review this checklist monthly (near the end of the month) to ensure that the home they are monitoring has turned in drills for each month. Spread sheet also serves as a reminder that sleep drills should occur in October and April. This is documented on the monthly site monitoring. See attachment E All Residential Supervisors and Program Specialists were re-trained on the fact that October and April are the months in which sleep drills need to occur within the organization.
