All required elements of the physical will be completed on an annual basis. The Program Specialist and Nurse will track appointments as they occur and note the annualized date to ensure compliance. If not earlier, 3 months prior to the necessary appointment, the Program Specialist (PS) and Residential Site Manager (RSM) will work with the health care provider to schedule the annual appointment. Once the appointment is scheduled, it will be placed on the residential calendar with the Program Specialist and Nurse receiving notice of the appointment. The Nurse will prepare the forms necessary for the appointment and provide it to the Residential Site Manager. The Residential Site Manager is responsible to ensure the individual is taken on the appointment and that all necessary paperwork accompanies the individual. Upon completion of the appointment, the paperwork will be scanned and submitted to the Program Specialist, the Nurse, and the Assistant Director. The form will then be filed in the ¿Medical¿ Binder located in the home for that individual. Both the Program Specialist and Nurse will review the form to ensure completion, and will notify the RSM if there are areas not completed on the form so the health care practitioner can be contacted for the missing information. If during the course of the annual appointment all necessary screenings are not completed, the PS/RSM will secure a second appointment to have these items completed and will ensure documentation is filed appropriately. |
