6400.70The home did not have an operable telephone. The telephone in the home did not work. There was no dial tone and the phone showed "no line".A home shall have an operable, noncoin-operated telephone with an outside line that is easily accessible to individuals and staff persons. The phones were working properly in the morning as several calls were placed/received. At the time of the inspection, the phone was not working. The agency¿s IT Department was notified, who contacted the service provider to determine cause. Comcast reported that due to an update they completed the number was wrong on their end and had to be fixed. The phones were operational within 40 minutes of the report. 02/28/2024 Implemented
6400.80(a)Outside walkways are not free from snow. The deck and steps off of the kitchen were not free of snow. Outside walkways shall be free from ice, snow, obstructions and other hazards. Cited concern rectified on January 23, 2024 bringing the home compliant with regulation. The Program Director will retrain all employees in this home, to include, the Program Manager and Program Coordinator, on regulation 6400.80(a) by February 28, 2024. Verbiage will be added on the agency audit form to support this regulation. Form will be revised and program leadership will be trained on the form revision by February 28, 2024. 02/28/2024 Implemented
