6400.68(c) | Coliform testing was done on 3/29/16, 6/23/16, 9/19/16, 1/11/17, and 3/17/17. The time frame between 9/19/16-1/11/17 exceeds the 3 month requirement. | A home that is not connected to a public water system shall have a coliform water test by a Department of Environmental Resources¿ certified laboratory stating that the water is safe for drinking purposes at least every 3 months. Written certification of the water test shall be kept. | To ensure compliance with 6400.68(c) homes not connected to a public water system have been set up to receive re-occurring appointments with Utility Management Services, INC. These appointments have been scheduled to occur at a minimum of every 3 months. Utility Management Services, Inc. will collect and analyze samples of water utilizing procedures as prescribed under the PA Safe Water Drinking Water ACT as it relates to detection of coliform bacteria in drinking water. The following location have been set up to receive re- occurring appointments: 21 Joni Drive Moscow Pa, 767 State Route 307 Spring Brook TWP Pa, 78 Keystone Road Clifton Township Pa, 44 Keystone Road Clifton Township Pa, 132 Main Street Gouldsboro Pa, 178 Main Street Gouldsboro Pa, 1197 Main Street Gouldsboro Pa, 28 Matthews Road Spring Brook Township and 119 Homestead Drive Moscow Pa. Results of each collection and analysis will be sent to and maintained by KCR Maintenance Department. |

6400.141(a) | Individual #1 had a physical exam on 1/27/2016. She did not have another physical exam until 3/8/2017, which exceeds the annual requirement. | An individual shall have a physical examination within 12 months prior to admission and annually thereafter. | Physical examinations will be reviewed monthly during each program¿s Clinical/Health Team Meeting. The current physical examination date along with the physical exam due date will be identified within the clinical / health team meeting minutes. During the meeting the team will review the physical examination due date to ensure the appointment has been scheduled in advance and will occur according to regulation. The program manager and or designee will be responsible for ensuring the appointment is scheduled and completed on time annually. All program managers, program coordinators, case managers, and area nurses have been made aware of the above process. The clinical / health team meeting note has been amended to reflect both the current date and due date for each consumers physical examination. |
