| 6400.141(c)(9) | The two most recent prostate examinations for Individual #1, date of birth 6/29/76 were completed 8/18/16 and 9/18/17. | The physical examination shall include: A prostate examination for men 40 years of age or older. | All community home nursing staff has been in-serviced on annual physical exams that prostate exams must be included for male clients over the age of 40. The director of health services will do quarterly random audits of the male clients over the age of 40. [Immediately, the director of health services will develop and implement a tracking system to ensure timely completion of physical examination with all required information, including prostate examinations. Immediately, the director of health services and all staff persons responsible for ensuring physical examinations are completed with all required information will be trained on the requirements of individuals' physical examinations as per 6400.141(c)(1)-(15).
