6400.163(h) | Methylphenidate 18 mg tablet to be taken daily by individual #1 was listed as expired on the blister tablet as of 9/30/2019. The medication listed as expired has been administered to individual from 10/1/19 through 10/15/19. | Prescription medications that are discontinued or expired shall be destroyed in a safe manner according to Federal and State statutes and regulations. | 1. Program Nurse contacted the pharmacy and confirmed the handwritten expiration date on the medication was an error. Attachment #9. The pharmacy sent a replacement. See attachment #10.
2. The Program Coordinator will complete a review of medications for all other consumers by 1/15/2020 and document compliance with 55 PA Code Chapter 6400.163 (h) on a Modified Self-Assessment Licensing Inspections Instrument. See Attachment #3. All areas of non-compliance will result in immediate corrective action and staff retraining.
3. In order to prevent reoccurrence, the Nursing Director retrained the Program Program Nurses on 55 PA Code Chapter 6400.163(h) by 12/19/2019. The training will instruct nursing staff to monitor expiration dates when checking in all medications. (Changing Practice). See attachment #11.
4. The Program Director trained staff responsible for the Plan of Correction and corresponding regulations on 12/13/2019. See Attachment #5. |
