Previous non-compliant practices of a coliform water test by a Department of Environmental Resources certified laboratory stating that the water is safe for drinking purposes at least every 3 months cannot be corrected. The previous Coliform testing was completed on 3/25/2016 and the most recent testing 6/13/2016 shows compliance in this area. See attachment #13 and #14.
2. The Program Director reviewed all sites requiring compliance 55 PA Code Chapter 6400.68(c) on 8/11/2016. Review consisted of confirming previous and most recent coliform testing was done at least every 3 months.
3. In order to prevent reoccurrence, the Program Director trained the Property Manager on 55 PA Code Chapter 6400.68c on 8/9/2016. See attachment 15. A spreadsheet will be used by the Property Manager to track coliform water test dates to ensure compliance. The Program Director will monitor during quarterly 1:1 meetings with the Property Manager to evaluate compliance. See attachment #16. |

2. In order to prevent reoccurrence, the Program Coordinator retrained all Program Supervisors (responsible for conducting all fire drills) on 4/14/2016 on 55 PA Code Chapter 6400.112(a) ¿was an unannounced fire drill held at least once a month.¿ See Attachment # 2.
3. The Program Coordinator will interview each staff present during a fire drill at each program by 9/30/2016 to track compliance. This will be documented on Attachment #1. |
