6400.67(b)Parts of the ceiling fan fixture in the rear living room were dangerously loose. When the pull chain to operate the fan's light was pulled, two pieces of metal making up the fan base separated, exposing wiring inside the of the fixture. The fan base remained in this state until staff on site pushed the two pieces of metal back together manually. Upon a second pull of the chain, the glass lightbulb cover came partially loose. The metal piece of the fan or the lightbulb cover may pose a hazard if the chain is pulled with sufficient force, as these parts of the fan fixture could come completely loose, fall, and cause injury. Floors, walls, ceilings and other surfaces shall be free of hazards.1. The maintenance department replaced the ceiling fan on 7/2/2021. [Attachment #6] 2. 6400.72(b)A window in Individual 3's bedroom was inoperable. When opened, the window slammed shut quickly; it was incapable of remaining in the open position. 6400.80(b)The wood of the exterior window frames was soft, splintering, and flaking paint. The outside of the building and the yard or grounds shall be well maintained, in good repair and free from unsafe conditions.1.
