The current Individual Support Plan (ISP) updated on 5/31/2023 and the annual assessment completed on 10/07/2022, document that Individual #3 has a hearing deficit and requires bed alarms/shakers to be alerted in the event of a fire. The bed alarm/shaker located in Individual #3's bedroom was not operable at the time of the inspection. | The home and the agency shall implement the individual plan, including revisions. | Individual JS did not return to the Lifesharing home following the licensing exit interview.
