There were crumbs and food debris in the top edges of the kitchen cabinet doors. The kitchen dining area had a sticky substance on the surface. There were two soiled kitchen towels hanging near the dining area that appeared to be wet and emitted a foul smell. The current Individual Support Plan (ISP) updated on 5/31/2023 and the annual assessment completed on 10/07/2022, document that Individual #3 has a hearing deficit and requires bed alarms/shakers to be alerted in the event of a fire. The bed alarm/shaker located in Individual #3's bedroom was not operable at the time of the inspection.The home and the agency shall implement the individual plan, including revisions.Individual JS did not return to the Lifesharing home following the licensing exit interview. JS was placed on Emergency Respite in an HRC CLA. The CLA has an interconnected fire alarm system. The bedroom that JS was placed in has a smoke detector in the room as well as a flashing strobe light that activates with the alarm system. There are also awake staff in the CLA at all times that will assist individual JS in evacuating the CLA. A new bed shaker was ordered and will be installed upon receipt.
