6400.64(a)The glass lighting fixture in the laundry room contained what appeared to be insect remains, which were visible when the light was in operation. The doorknob on the door in the rear bathroom that leads to an adjacent bedroom was roughened and discolored in a manner that suggested it was corroded by exposure to moisture over a long period of time.Clean and sanitary conditions shall be maintained in the home. 6400.67(b)The rear bathroom's shower rod had metal ring fasteners connecting the shower rod to the wall. One of these metal fasteners was brown and flaking over the entirety of the ring, in a manner consistent with rust. 6400.32(r)Individual 1's, Individual 2's, and Individual 3's bedroom doors did not have locking mechanisms to afford them privacy within their bedrooms. According to Staff on Site, none of these individuals had consented to removing the locks and the removal of locking mechanisms had not been discussed with the individuals' Support Team members.An individual has the right to lock the individual's bedroom door.All individuals teams have been contacted to discuss and consent to the removal of locking mechanisms due to safety surrounding medical and or behavioral issues. All Individuals Annual assessments have been addended to include the information and team agreement form has been sent to team members for signatures documenting consent for removal of locks as it has been hard to get signatures on the addended assessment with many working Virtual.
