| 6500.17(b) | The 6/16/21 Self-Assessment did not measure the following regulations- 79c, 79d, 79e, 79f1, 79f2, 79f3, 79f4, 79f5, 79h, 80a, 80b, 80c, 80d, 81a, 81b, 82. | The agency shall use the Department's licensing inspection instrument for this chapter to measure and record compliance. | Immediate fix: The Life Sharing Specialist will complete the entire self-assessment no later than 8/27/21. The Team Supervisor will also sign the self-assessment to verify completion.
Correction:
1. Program Director, will train all Team Supervisors on the regulation no later than 8/27/21.
2. The Team Supervisors will train all Life Sharing Specialists that did not complete the self-assessment correctly no later than 8/27/21.
3. The self-assessment that had the violation will be corrected by 8/27/21.
4. Program check not needed since all self-assessments were already reviewed during inspection.
Attachments:
1. Attachment Log Sheet to organize all the attachments with a reference number (Attachment #1). A copy will be sent to ODP no later 9/30/21.
2. Training Summary Form for all Team Supervisors (Attachment #2). A copy will be sent to ODP no later 9/30/21.
3. Training Summary Form for all Life Sharing Specialist that were out of compliance with this regulation (Attachment #3). A copy will be sent to ODP no later 9/30/21.
4. A copy of the corrected self-assessment including the Team Supervisor signature will be sent to ODP no later than 9/30/21 (Attachment #6). |
