The Program Specialist reviewed the revised ¿Rights¿ with Individual #1 and obtained a signed copy on 11/4/2015. The Records Technician sent the revised rights form to their family/guardian (attached). The Records Technician will send all individuals and their guardians a copy of the revised rights form at the annual update cycle date in January 2016. |

6400.62(a) | On 10/22/15 at approximately 9:20 AM the cabinet under the kitchen sink was unlocked and contained the following poisonous materials: Majestic triple action multi-enzyme spotter; and Lock fill degreaser. Both containers stated "if swallowed contact poison control." Individual #1's assessment dated 9/8/15 indicates s/he is not safe around poisons. | Poisonous materials shall be kept locked or made inaccessible to individuals. | The Program Specialist locked The cabinet immediately upon discovery that it was opened. The Program Specialist will review with all staff at the next Program Review meeting on 11/18/15. The Community Home Coordinator and Program Specialist in all homes will monitor to ensure the cabinets are locked by checking it throughout the day.[The community home coordinators and program specialists will educate all staff working in community homes on the requirement that all poisonous materials are kept locked and the agency¿s policies and procedures on doing so by 2/15/16. The community home coordinator and/or program specialist will conduct unannounced visits to every community home at least weekly for the next two months upon receipt of this document and physically check if all poisonous materials are locked and policies and procedures are being implemented.
