| 6400.181(a) | Individual #1 had an assessment completed on 3-22-18 and then again on 4-10-19. | Each individual shall have an initial assessment within 1 year prior to or 60 calendar days after admission to the residential home and an updated assessment annually thereafter. The initial assessment must include an assessment of adaptive behavior and level of skills completed within 6 months prior to admission to the residential home. | Training/review was completed on 9/11/19 with supervisor responsible for assessment that was completed out of compliance. Timelines were reviewed for compliance and per regulation requirements by Assistant Program Director Matt Smeltzer. On 9/26/19, all residential program supervisors will be retrained on initial and annual assessments and the requirements and time frames in which they must be completed. All initial and annual assessments will be tracked and reviewed by the Assistant Program Directors and then they will be forwarded to Licensing Coordinator.
