6400.112(d)Individual #1 did not evacuate during the fire drills held on 6/28/2023, 7/24/2023, 8/29/2023, and 9/27/2023. Individuals shall be able to evacuate the entire building, or to a fire safe area designated in writing within the past year by a fire safety expert, within 2 1/2 minutes or within the period of time specified in writing within the past year by a fire safety expert. The fire safety expert may not be an employe of the home or agency. Staff assistance shall be provided to an individual only if staff persons are always present at the home while the individual is at the home. 6400.163(c)The psychiatric medication review for Individual #1 completed on 1/9/17 did not include the necessary dosage of the medications. If a medication is prescribed to treat symptoms of a diagnosed psychiatric illness, there shall be a review with documentation by a licensed physician at least every 3 months that includes the reason for prescribing the medication, the need to continue the medication and the necessary dosage.The medication review form has been updated in section; "Current Medications prescribed by Psychiatrist only" was changed to "Current Medications including dosage and time prescribed by Psychiatrist only. The Health Services Coordinator will review the changes to the form with the supervisors at Res team meeting on 10/18/17 who will then address the change at staff meetings to make all staff aware of the need for the dosage and time to be documented on the review form. This is now in the medication management plan and was implemented on 9/29/14. The Purple form called "Medication Change Alert" has been in place for a long time and is part of tracking on the log also. This information will be reiterated to all staff through a memo.
