6400.144 | Repeat 10/24/22- Individual #1's physical dated 2/15/23 states that they are to have a caffeine-free diet. Per the individual's financial logs, there are many times each month (easily 10 a month) where the individual is purchasing soda from fast food locations. The soda that the individual is purchasing is not caffeine free as confirmed with the Program Specialist and Individual. There is no documentation that the individual has been educated to maintain a doctor recommended diet. | Health services, such as medical, nursing, pharmaceutical, dental, dietary and psychological services that are planned or prescribed for the individual shall be arranged for or provided.
| The individual does purchase sodas from fast food locations and opts for caffeine free. Attachment 8a is the tracking sheet to log the individual¿s outings. It shows that he orders, sprite, or caffeine-free soda, or root beer, etc. Attachment 8b is a receipt for a purchased drink ¿ a Sprite. The receipt does not list the particular soda but the brand name, i.e. Pepsi. Because a receipt lists the name brand versus an actual item name, his financial logs look as though he never orders a caffeine-free drink and are misleading. As the Activity Chart shows, a line encouraging the individual to choose diet caffeine-free products has been added. The individual has the choice of what to order and, when necessary, staff encourage him to make a good choice. The violation states that at least 10 times per month he purchases a soda, which is true. However, as the receipt does not specify what kind of drink, one cannot come to the conclusion that all his drinks are never caffeine-free. |
