| 6400.64(a) | On 7/7/23 at 12:29PM, there were multiple areas of a black spotted substance that appears to be mold and/or mildew, on the ceiling and walls along the perimeter of the ceiling line, in the hallway bathroom on the first floor of the home. | Clean and sanitary conditions shall be maintained in the home. | Immediate Corrective Action: Maintenance Director, scrubbed the areas which had mildew in the bathroom, primed and painted the area on July 7, 2023.
All other sites were checked for any mold / mildew and other sanitation issues between July 20th and July 24th and were determined to be compliant. Responsible Party , CAO |
