6400.72(a)The window on the left of the central picture window nearest to the dining table in the kitchen was open approximately 2 inches. The screen was separated from the window frame leaving a gap from 1 to 2 inches along the right side. Windows, including windows in doors, shall be securely screened when windows or doors are open. Window was shut on site. Staff will receive training to ensure that when using windows for ventilation or fresh air that the screens will be checked to ensure they are securely screened before leaving them open. This specific issue, and all physical site issues that were identified as non-compliant will be reviewed thoroughly when training on the new 6400 regulation checklist process. CLC¿s process for ensuring ongoing compliance with the 6400 regulations 61A-86 pertaining to the physical site has been revised to prevent this issue in the future. Our process was daily and required a daily check by the supervisors ¿ a single check mark indicating that all items were compliant. The physical site checks are now weekly and each inspection item has been written out on the form with the expectation of checking for compliance and marking compliance for each item each week. Inspections are to be completed by the Residential Program Supervisor and submitted each week to the Program Specialists along with any corresponding corrections or work orders. (Checklist sent 5/13) Residential Program Supervisors received training on May 12, 2016 for implementation of this process, which is being implemented starting May 15, 2016. Supervisors absent from the meeting and the remaining residential staff will receive training between May 13th and May 23rd. 6400.72(b)The two side windows adjacent to the central picture window nearest to the dining table in the kitchen had screens that had duct tape around the perimeter which was loose and separated from the window frame leaving a 1 to 2 inches gap on the right side of both windows. Screens, windows and doors shall be in good repair. The screens were replaced the day of the onsite visit by licensing. Pictures were provided at that time. All homes were inspected between May 4-5, 2016 by Barb Kowalsky and Ed Kuznik. Screens at other sites were in good repair. 6400.101The doorway leading from the kitchen to the porch on the side of the house was obstructed by a metal bench on the porch.Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed. It has been reiterated to all staff at the site to keep stairways, halls, doorways, passageways and exits free from all obstructions. All staff at the site have been retrained on this requirement. [As per conversation with the Director of Program Development, the 3 program specialist assigned to each home along with the residential supervisors of each home will do an on-site monitoring at each community home and check for obstruced egresses and will address immediately.
