| The missing screen in the downstairs bathroom was replaced by COMHAR¿s maintenance department. Ventilation issues at this site and all COMHAR sites will be reported in COMHAR¿s maintenance reporting system by the Assistant Site Manager assigned to the site. The work ticket generated by the report will be assigned to a maintenance mechanic by COMHAR¿s Maintenance Technician. Repair or replacement will be addressed by the assigned maintenance mechanic. Quality assurance will be provided by the Assistant Site Manager with oversight provided via the assigned Site Manager. |

| Individual #1¿s Consumer Face Sheet was completed to include his religious affiliation. All items on the Consumer Face Sheet will be completed by the assigned Site Manager at intake of new individuals. The Consumer Face Sheet will be updated by the Site Manager as information changes. Chart audits by the Assistant Residential Directors will assure that this form which contains all required demographic information is complete. Staff were also retrained on this process |
