6500.17(a) | The self assessment of the home was not dated; therefore, compliance could not be measured. | If an agency is the legal entity for the family living home, the agency shall complete a self-assessment of each home the agency is licensed to operate within 3 to 6 months prior to the expiration date of the agency's certificate of compliance, to measure and record compliance with this chapter. | Self assessments of the family living homes will be completed in the 3-6 month window prior to inspections. They will be completed, and filled out in its entirety and dated before placing in the licensing book by the Program Specialist Monica Gardner. The licensing book will be reviewed Dana Zinck assistant ex-director quarterly to make sure that everything is accurate.[Upon receipt of the agency's certificate of compliance, the CEO shall review the expiration date and develop and implement a procedure to ensure the that the agency completes a self-assessment of each home within 3 to 6 months prior to the expiration date of the agency's certificate of compliance. Within 1 month prior to 3 months before the expiration date of the agency's certificate of compliance the CEO or designee shall audit the self-assessments for each home to ensure timely completion.
