Drawers are not able to open and close properly. | Furniture and equipment shall be nonhazardous, clean and sturdy. | A dresser drawer in an individual's room could not be opened during inspection. This was due to the drawer being repeatedly being forced open and closed because of the amount of clothes in it. A work order was placed on 2/4/2021 (5A), and the track was fixed on the same day, 2/4/2021. Also on 2/4/2021, staff and the individual went through her clothes in the drawer and removed items she didn't want. This lessened the amount in the drawer so it can now be easily opened and closed. |

| 6400.163(h) | Prescription for individual #9's current medication listed on MAR, ammonium lactate 12%, expired on 8/14/19. | Prescription medications that are discontinued or expired shall be destroyed in a safe manner according to Federal and State statutes and regulations. | Expired foot cream was found in the medication box for one of the individuals. This occurred because Site Supervisor was not checking the medications that weren't in a blister pack. On 1/20/2021, Site manager returned the expired foot cream to Elwin Specialty Care pharmacy (5C). |
