6500.62(a) | Individual #1 is not safe with poisons and they need to be locked, according to the Individual's current Individual Support Plan (ISP). A bottle of Pine Sol multi-surface bathroom cleaner and a bottle of Lysol Power bathroom foam cleaner were found in an unlocked cabinet in the second floor bathroom. Both products were labeled with the instruction to "contact poison control if ingested." | Poisonous materials shall be kept locked or made inaccessible to individuals. | 1. The poisonous products were removed on day of licensing, 3/2/2022, from the bathroom. They were placed in a locked cabinet with the other household cleaning products.
2. The provider and LS coordinator received retraining on poisonous material hazards in regards to the individual's health and safety abilities and risks. The date of the retraining was 3/16/2022. The 6500 regulations were also reviewed.

The landline phone has been replaced and is operable. There were operable cell phones in the home at all times for the individual's use and for emergencies. A video of the operable phone will be emailed to Chris Hadley with the other supporting documentation.
2. The provider and coordinator received retraining on the importance of having a functioning landline phone in the home as well as on the communication needs of the individual as it pertains to the individual's health and safety risks.
