6500.125(c)(2)Staff #1 began providing services in her house on 6/11/2019. Staff #1 does not have a TB test. There is a chest x-ray on file dated 1/11/2016. Her physical exam dated 3/17/2019 does not state that she's tested positive for TB.The physical examination shall include: (2) Tuberculin skin testing by Mantoux method with negative results every 2 years for family members 1 year of age or older; or, if a tuberculin skin test is positive, an initial chest X-ray with results noted. Tuberculin skin testing may be completed and certified in writing by a registered nurse or licensed practical nurse instead of a licensed physician.1. The provider (MP) did not have an initial chest x-ray with results noted in her provider file at time of inspection. The provider applied to Access Services in March of 2019 to provider host family services. MP had a complete physical on 3/17/2019 with a physician noting that MP had a chest x-ray in lieu of a PPD on 1/11/2016. Our intake coordinator believed that to be compliant. 2. On 12/2/2019 the provider¿s physician faxed to Access Services the reason why MP receives chest x-rays in lieu of the PPD test. Due to MP receiving the BCG vaccine against TB while in her native country outside of the United States, she is predisposed to having a false-positive reaction to the PPD. Thus the provider receives chest x-rays. Records of her initial chest x-ray were not available to her doctors in this country. 3. The intake coordinator will ensure that moving forward we will get the results of the initial positive chest x-ray for all applicable providers. 12/02/2019 Implemented
