There was a telephone that did not have a dial tone and it was not possible to call out from the telephone.A home shall have an operable telephone that is easily accessible.1. On date of inspection the provider was informed that her phone had no dial tone. The provider currently receives landline services through RCN/Astound. The provider does have cell phones in the home for the consumer to utilize until a new phone could be purchased. A new phone was ordered through Amazon and was delivered on 4/22/2022. It is functioning at this time. 2. The provider and coordinator received retraining on the importance of having a functioning landline phone in the home as well as on the communication needs of the individual as it pertains to their health and safety abilities and risks. The door to Individual #1's bedroom did not have a locking mechanism.An individual has the right to lock the individual's bedroom door. Locking may be provided by a key, access card, keypad code or other entry mechanism accessible to the individual to permit the individual to lock and unlock the door.1. The individual's team members met on 3/10/2022 to discuss with the individual again his desire to have a lock on his bedroom door. At the time of the meeting the individual restated that he did not want a lock on his door. The SC amended the ISP to reflect the individual's desires. 2. The provider and coordinator received retraining on the individual's rights and preferences as it pertains to his health and safety needs. The retraining occurred on 3/10/2022.
