6500.122(a) | Individual #1 had a dental exam on 6/16/2015; she didn't have another dental exam until 7/12/2016, which exceeds the annual requirement. | An individual 17 years of age or younger, shall have a dental examination performed by a licensed dentist semiannually. Each individual 18 years of age or older shall have a dental examination performed by a licensed dentist annually. | 1. Though the individual did not have a clinical dental exam in a timely manner, it should be noted that she was seen a dentist on the following dates; 6/16/15, 7/17/15, 10/30/15,11/6/15,7/12/16, 8/19/2016 and 1/17/17. An appointment was scheduled for an exam on 6/29/2016 but was cancelled by the dentist. The soonest they could reschedule the appointment for was on 7/12/2017.
