| 6400.195(a) | Individual #5's Behavior Support Plan (dated 1/17/2022) included a restriction that was not reviewed by the human rights team prior to the use of this restrictive procedure. | For each individual for whom a restrictive procedure may be used, the individual plan shall include a component addressing behavior support that is reviewed and approved by the human rights team in § 6400.194 (relating to human rights team), prior to use of a restrictive procedures. | The plan stated that an individual's toys would be kept locked away and be given to him when he could be supervised. This component of the plan was never implemented, and the toys were not locked during inspection. The restrictive component of the plan was recognized and corrected in the behavior plan dated 8/5/2022. The plan had not been filed in his home at the time of the inspection. It is now filed and the staff trained. |
