The wall behind the kitchen stove is painted rather than a backsplash and is difficult to clean. The wall was cleaned as best as it could be and a work order was put in to the apartment complex requesting the installation of a backsplash. We have not had a reply to that request at this time. Cleaning was discussed with Program Coordinators and in addition to frequent inspections, Program Coordinators have been instructed to review the cleaning supplies used in their homes to insure that the cleaning products are appropriate to the cleaning task and issues (ie: grease vs soap scum) 09/18/2017 Implemented

A computerized system has been put in to place to insure staff update their physicals in a timely fashion. Staff with outdated physicals will be removed from the schedule and documentation maintained to indicate they were not working until a current physical is received. 6400.112(d)Individuals that live in this home evacuate to a fire tower. There was no documentation to determine if the fire safe area (fire tower) had been approved by a fire fire safety expert.(d) Individuals shall be able to evacuate the entire building, or to a fire safe area designated in writing within the past year by a fire safety expert, within 2 1/2 minutes or within the period of time specified in writing within the past year by a fire safety expert. The fire safety expert may not be an employee of the home or agency. Staff assistance shall be provided to an individual only if staff persons are always present at the home while the individual is at the home. A fire safety expert had approved the fire safe area previously, but the documentation was not in the past year. It is important to continuously insure that the fire safe area is properly maintained for the safety of the individual. The Assistant Program Director contacted the fire safety expert who approved the fire safe area on March 27, 2013. A second fire safety expert was also contacted to insure approval of the area can be received annually. Additionally, this documentation will be maintained with the fire drills in the office and reviewed monthly with the fire drills to insure the approval does not expire. 03/27/2013 Implemented
